(a) Requirements for a new passport on completion of validity
- Prescribed Application Form
- 4 passport-sized recent photographs
- Current passport
- Copy of Lao Identity Card
- Fees: US$ 40 in case of normal passport booklet, US$ 50 in case of Jumbo Passport Booklet (containing
60 pages)
(b) Requirements for duplicate passport in lieu of lost or damaged passport
- Passport fees: US$ 125
- Prescribed Application Form
- 8 recent passport size photographs
- Photocopy of the lost passport and the passport in original in case of damaged passport
- Police report, in original, stating loss of passport
- Proof of residence in Lao PDR
- A written statement explaining circumstances, in which the passport is lost / damaged
- In urgent cases, two separate affidavits from two Indian nationals certifying applicant's identity, etc., are also required. Fee for each attestation is US $ 10/-. (this applies in case-to- case basis)
(NOTE: Penalty for the loss or damage of the passport will be separately charged and decided by Embassy/Consulate on case-to-case basis.)
(c) Requirements for passport for child less than 15 years of age
- Prescribed Application Form
- 4 recent passport size photographs
- Birth certificate in original along with a copy
- Passports of parents along with copies
- Proof of stay
- An affidavit signed by both parents that they have not applied for any other nationality for the child
- Fees for the issue of passport: US $ 25/-
- In case, birth registration certificate for minor issued by the Embassy /Consulates, an extra fees of US $ 20/- is to be paid
(NOTE: This passport will be valid for five years.)
(d) Change of name in passport following marriage, re-marriage or divorce
- Prescribed application form
- Passport in original
- Attested copy of marriage certificate issued by the Registrar of Marriages
- Copy of spouse's passport
- Copy of residence permit, in case the same is not endorsed on the passport
- In respect of change of name by a male applicant, the new name must be published in a newspaper, both in Germany and in India in the area of his residence and the newspaper cuttings, in original, from both the newspapers must be submitted along with the application form
- Applicant is required to sign an affidavit in the presence of Consular Officer. Fees for the affidavit is US $ 10
- A fresh passport will be issued and the fee will be US $ 20/- if validity is restricted to existing passport and US $ 40/- in case validity is given for 10 years

(f) Fee for other miscellaneous services
- Fees for miscellaneous service, which require issue of a fresh passport booklet is US $ 20 in case of normal passport booklet containing 36 pages and us $ 25 in case of Jumbo passport booklet containing 60 pages
(NOTE: The changes, which necessitate the issue of fresh booklet, are change of name, change of photograph, change in date or place of birth, change in signatures, address, parentage etc.)
(g) Other relevant information
Jumbo Booklet
- Frequent travellers can now obtain 60 page Indian Passports.
- The charge for the ten-year-validity Jumbo Passport booklet is US $ 60/-
- Ordinary passport booklet (36 pages) with a maximum validity of ten years costs US $ 40/-
Additional visa sheets discontinued
- Additional visa sheets for attachment in passports will no longer be issued. Those holding passports with existing validity, but with all pages exhausted, will now be issued with an additional passport booklet. Fees for ordinary additional booklet is US $ 20 and for Jumbo booklet is US $ 25.
- (Documents to be submitted for this purpose will be the same as for the issue of a new passport on completion of validity)
Re-issue of passports within one year before expiry
- It is possible to have a new passport, one year before its expiry.
- This passport will be valid for 10 years from the date of issue. Fees in this case will be same as fees charged in case of issuing new passport on completion of full validity.
Discontinuance of 20 years passport booklet
- 20 years valid passport booklets are no longer issued.
